How to Get to the Midleton Medi Centre
Midleton is communicable via Public Transport such as Train/ bus and our practice is located in the town centre within walkable distance or Taxi from these sites. There is a map located at the bottom of the page.

New Patient Registration
To Register as a new patient, please contact our reception staff on 021 4632288. They will ask you to complete our practice registration form and also to sign an authorisation form so that you medical records can be transferred from your previous GP.
Please note each month there is a limited number of patients that we can register at our practice due to our availability of staff and ensuring the highest standards of medical care.
If you hold a medical card/ doctors visit card please insure all the appropriate details are up to date.
How do I order a repeat prescription?
To order a repeat prescription please phone our practice
New medication requests or changes or any other clinical query will need to be dealt with in person with a doctors review.
Please note, in line with best medical practice and your safety certain medication requires more frequent monitoring with blood tests and GP reviews.
We do our best to complete prescriptions on the same day but please allow 48hours from request to be sent to your nominated pharmacy.
If your prescription is requested on a Friday please allow 72 hours
How do I enquire about my blood results or imaging results?
Once your bloods or imaging test has been undertaken in our practice, we aim to contact you by text or call as soon as results are received (usually within 2 weeks, note some special test may take longer).
It is of paramount importance that you follow up with us after 2 weeks if you have to got your results.
You must opt-in to receive texts from the surgery and complete the text message consent form here.
Note results of tests that were ordered by your consultant do not routinely come back to the surgery and please contact your consultant if you have not received your results.
How do I extend an illness certification?
- If you are employed and you are unable to work due to illness or injury, it is important to speak to your employer to find out what their procedure is under the Statutory Sick Pay Scheme.
- If you’re unable to work due to illness, you may be eligible for Illness Benefit from the Department of Employment Affairs and Social Protection (DEASP). To apply, you’ll need a ‘Certificate of Incapacity for Work’ from your GP. Your GP can electronically submit this certificate through a secure online system.
- Sometimes, one certificate can cover the duration of your illness, but if not, you’ll need to return for further review.
- You will also need to complete a separate application form (IB1), which you can collect from the surgery. Alternatively, if you have a MyGovID Account you can apply online here.
- If you are not claiming Illness Benefit and you require a sick certificate for your employer there is a fee of €20 payable in advance.
- Remember, you must visit a GP to receive either an illness benefit certificate or a medical certificate. If you need to extend either certificate, you can contact us and we’ll advise you if a GP review is necessary.
How do I claim medical expenses?
- if you have a private medical insurance company, you can contact your provided directly (VHI, Quinn, Aviva) via their website/ app – links below and fill out their individual claim forms.
- Typically you can upload your receipts via your insurance providers portal and they will allow you to claim then. For full details please visit their sites below
- You can also claim Health Expenses via Revenue.ie
- The Med 1 form is no longer available.
- Health expenses are claimed through your Income Tax Return. If you are a Pay As You Earn (PAYE) taxpayer, you also have the option to claim relief in real time during the year.
- You can claim relief on health expenses through myAccount or Revenue Online Service (ROS).
- You can only claim for expenses that you have receipts for. You can claim relief on the last four year’s health expenses.
